We are currently hiring CNAs for our morning and evening shift. The morning shift is from 7a.m. to 3 p.m. The evening shift is from 3 p.m. to 11 p.m.
Certified Nurse Assistants (CNA) provide each resident with routine daily nursing care with care compassion and respect. This care is also in accordance with the resident’s assessment plan and with current federal, state and local standards and regulations.
Our CNA’s are empowered to “do the right thing”, make sound independent decisions as they demonstrate patience, initiative and willingness to assist residents.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Ensuring that all nursing care is provided in privacy
- Making residents comfortable by attending to their everyday needs including bathing, grooming,
walking, sitting, night time routines, water/food needs, etc. - Assisting residents transporting them as needed including meal times, activities, physical
therapy, etc. - Getting to know the residents, monitoring their wellness and communicating any changes to
Management. - We take confidentiality very seriously …. Maintaining confidentiality including HIIPA, passwords,
User codes, etc. is critical.
REQUIREMENTS
- Education/Licensure
- High school diploma or GED
- CNA training completion from accredited school
- Valid CNA certificate number
- Qualifications/Experience
- Prefer 1 year work exp
- Experience and computer experience, however, it’s not necessary. We provide
on the job training - Must be able to speak, read, write and comprehend the English language

Topanga Terrace is hiring for an RN Supervisor for Day and Night Shift
Objective:
The primary purpose of your job position is to supervise the day-to-day nursing activities of the facility during your tour of duty. Such supervision must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility, and as may be required by the Director of Nursing Services
Essential Duties and Responsibilities:
- Assessment and development of resident care plans
- Ensuring that all nursing care is provided in privacy
- Checking vital signs
- Dressing wounds
- Passing out medications
- Assisting with admissions
- Making rounds with physicians
- Starting IVs
- Listening to residents and their families
- Addressing questions/concerns of residents
- Performing accurate and detailed charting of resident progress notes
- Providing assistance to LVNs and CNAs
- Ensuring equipment is in good operating order
- Monitoring residents that are at risk for falls
- Collecting lab samples
- Abiding with all facility policies and procedures including not disclosing user ID codes and passwords
- Following Infection and Control policies
- Reporting any occupational exposures to blood, body fluids, or other hazardous materials to a supervisor
immediately - Attending annual facility in-service training programs
- Completing other assignments when requested by management
Required Education / Licensure:
Nursing Degree from an accredited school or college
Valid RN License in good standing
Valid CPR and BLS Card
Experience/Qualifications:
Minimum 1 year experience in skilled nursing experience preferred
Must be able to speak, read, write and comprehend the English language
Ability to use computer or tablet to enter resident data
Physical Requirements:
Must be able to move intermittently throughout the day
Working throughout the nursing areas
Must be able to lift up to 25 pounds. Lifting more weight up to 100 pounds requires assistance
Ability to read fine print on tablet, progress notes and/or medical labels

Topanga Terrace is hiring for a LICENSED VOCATIONAL NURSE (LVN) for all shifts, part time or full time.
Objective:
The primary purpose of the Licensed Vocational Nurse (LVN) is to coordinate, evaluate, and assist in the
delivery of Nursing care and services. You are also responsible for managing the residents’ care plans and
supervising resident care activities. Both care management and supervisory responsibilities must be
executed in accordance with state and federal regulation and facility policies and procedures.
Essential Duties and Responsibilities:
- Direct day-to-day resident care activities.
- Supervise the Certified Nursing Assistants’ delivery of care and proper implementation of the
standard of care. - Complete medical forms, reports, evaluations, studies, charting, etc. as needed.
- Participate in the facility’s quality assurance (QA) program as requested or needed.
- Ensure that reference materials (Regulations, Policies and Procedures, etc.) are maintained and
organized at the nurses’ station. - Admit, transfer, and discharge residents.
- Review and complete incident/accident reports and conduct investigations as required.
- Interpret facility policies and procedures to staff, residents, visitors, and government agencies as
required. - Attend various facility meetings (care conferences, stand-up meetings, nurses’ meetings,
Medicare meetings, discharge plan meetings, etc.) as required. - Receive telephone orders from physicians, process them in accordance with facility procedures,
and update the care plan as required. - Report all care ordered but not delivered, medication errors, charting errors, diet changes, etc.
to the Director of Nursing. - Complete medication and treatment passes as applicable in a timely manner and in accordance
with state and federal regulation and facility policy. - Periodically review medication and treatment administration records for completeness and
accuracy and report any discrepancies to Director of Nursing. - Inform nursing staff of new admissions, including expected time of arrival, room assignment, etc.
- Make daily resident rounds to observe and evaluate the resident’s physical and emotional status.
- Meet with residents and/or family as needed.
- Notify attending physician, family, and members of interdisciplinary team when resident is
involved in an incident and/or accident or if there is a change in condition. - Consult with attending physician as needed for treatments, medications, etc.
- Participate in the facility infection control program for monitoring communicable and/or
infectious disease among residents and staff. - Participate in procedures for reporting hazardous conditions or equipment.
- Partner with Director of Nursing to ensure staff coverage requirements are met.
- Give and take report upon beginning and ending of shift.
- Other duties as assigned
Required Education / Licensure:
